Abstract
|
|
|
Location of gastric ulcer was examined based on total 736 cases of gastric ulcer found endoscopically using Olympus GF-C, GF-B, JFB and GIF-P during the period of 9 years and 9 months from Jan. 1968 to Sept. 1977 at the Endoscopy Room of Kyungpook University Hospital in Taegu,. Korea. The material included 543 cases of benign single gastric ulcer, 112 cases of multiple gastric ulcers and 81 cases of malignant ulcer in which possibility of benign ulcer was also considered as a diagnosis. of second choice.
The results ere summarized as follows:
1. The Beak age of the benign single ulcer was in 4tlz decade while that of the benign multiple ulcers was in 3rd. Concerning the location of the ulcer, 62.1 0 located at the middle third of the stomach in single ulcer cases while, 53.5% located there iri case of multiple ulcers and incidence at the distal third of the stomach was considerably higher in the latter.
2. The peak age of the malignant ulcer was in 5th dec,~de and its incidence was highest also at the middle third of the stomach but discrepancy between the incidence at the middle third and that at the_ distal third is much smaller than in case of benign ulcer.
3. As the age goes up, the incidence of benign single; ulcer at the proximal third of the stomach increased while that at the distal third decreased. However, in cases of malignant ulcer no apparent relationships between the ulcer incidence by location of the stomach and age of the patient could be seen.
4. Among total 257 benign looking ulcers on whom end´.oscopical biopsy, cytology or surgery was performed, 20cases(7.8jo) was proved malignant acid misinterpretation by endoscopy was highest in the distal third of the stomach (22.0%)- On thy: other hand, among the 99 malignant looking ulcers on whom endoscopical biopsy, cytology or surgery was done, 18cases(18.2%) was failed to be shown malignant and the misinterpretation by endoscopy was highest in the proximal third of the stomach(36.8%).
